Dental hygiene care plan uses all of the information of the dental hygiene diagnosis and establishes goals, interventions conditions/criteria/outcomes for follow up and recare dates.
Developing the Dental Hygiene Diagnosis
The client you will be evaluating has the following data:
Male, 49 years old, smokes 15 cigarettes daily
Medical history : low dose aspirin 81 mg and lisinopril 10mg daily. Allergy to Amoxicillin
Chief Complaint: sensitive teeth when brushing and flossing. Upper lip hurts.
Oral exam: lesion on upper lip is fluid filled.

The GOALS will now address the the problem by assigning a goal of remineralizing enamel. What recommendation in the intervention will you make? Key ideas will take into consideration where the decalcification is located and dietary analysis result.
The INTERVENTION determines what is recommended to accomplish the short term goals.
The CONDITIONS/CRITERIA/OUTCOMES determines the reinforcer for the achieved goals, and measurable index used.

According to CAMBRA guidelines client is high risk for caries.
Soft Tissue Diagnosis:
vesicle on upper lip--- related to------ herpes virus.
Periodontal Diagnosis:
localized chronic perio with generalized recession------------- related to---------- bacterial plaque/biofilms.
Now determine the goals related to the problem diagnosis
Goal:
Address plaque, eliminate smoking habit,remineralize enamel ,improve tissue, gingival texture, reduce bleeding.
Intervention:
Implement CAMBRA, mechanical toothbrushing, flossing/interdental brush, habit control therapy for smoking cessation Prescription: Prevident 5000 Booster toothpaste, Prescription: denavir1% for herpetic lesion.
Reinforcer: Client will go out to dinner when goals are met.
Criteria/Outcome:
Plaque will be reduced at next recare with measurable index(MI) by plaque photos.
Smoking will be eliminated at next recare with MI by verbal information.
Enamel will be remineralized at next recare with MI by dental chart.
Herpetic lesions will be controlled at end of current recare with MI by visual inspection.
Bleeding will be eliminated at next recare with periodontal chart.
The outcomes are evaluated at the next recare with the time interval determined by the client need.
